After completing the onboarding process for a new employee ๐ง๐ผ, the next important step is to obtain their medical insurance card ๐ฅ.
Step 1
The employee must complete the Declaration Form provided by Tawuniya Insurance Company. All family members, including spouse and children, should be included in the list of dependents.
Step 2
The company should stamp and sign the completed Declaration Form before sending it to the health care provider.
Step 3
Attach all supporting documents when submitting the accomplished form, as follows:
- Personal Identity Card (National ID)
- Residence Identity Card (Foreign National)
- Family Identity Card
Step 4
Request a pro forma invoice after you have submitted the accomplished Declaration Form along with supporting documents to the health care provider contact person.
Step 5
Share the pro forma invoice for the outgoing payment with your accounts team.
Step 6
Share the proof of payment with the insurance company after you receive confirmation from your accounts team.
Step 7
Visit the CCHI website (www.eservices.cchi.gov.sa) to verify if the candidate has active medical insurance from their previous company.
Step 8
Enter the employee information along with the code to display the details.
